PCUD Application for Service information

The undersigned applies for gas service at the address below and agrees to pay for service as measured by the district’s meters according to rates applicable. Applicant agrees to permit district’s agents free access to property of the district. District shall have the right, but no obligation, to inspect any installation before or after service is introduced and reserves the right to reject any wiring, piping, or appliances not in accordance with the district’s standards. But such inspection, failure to inspect or reject shall not be regarded as insurance against defects in installation, wiring, piping, or appliances and shall not render the district liable for any loss or damage resulting from defects in same, or from violation of ordinances, rules or regulations, now in force or as may be adopted, or from accidents which may occur on customer’s premises.

Applicant agrees to hold the district, its employees, agents, successors, and assigns harmless from any and all claims for damages to person or property arising from or in any way related to the undersigned’s failure to disclose the types and locations of underground utilities or other services prior to the commencement of installation of gas service, and agrees to indemnify the district for any and all cost incurred from such claims including attorney’s fees. The District will, subject to the execution by applicant and acceptance by the district of a service contract, furnish and install a main or service for payment of a tap fee. The tap fee is non-refundable and entitles the consumer to 200 feet of line. However, should the installing of said line require expenditure in excess of the tap fee, the district reserves the right to stop and/or refuse installation unless customer agrees to pay for the excess cost. Applicant agrees to pay applicable excess footage charge for line beyond 200 feet. Applicant certifies that the meter fee indicated below is not a deposit and is non-refundable. In the event a security deposit is required, it will be returned when all bills have been paid and service discontinued. Applicant understands a minimum bill will be charged 30 days after service is installed even if no gas is being used. Applicant agrees that this application is subject to the district’s rules and regulations, present and future, which are incorporated herein by reference as fully as if copied verbatim, copies of which may be inspected at the district office.
